POSITION SUMMARY:Supports the Department by performing diversified administrative, organizational and clerical duties.

ESSENTIAL DUTIES AND RESPONSIBILITIES 

  • Call patients 30 days post discharge.
  • Continued follow-up with discharged patients every 30 days.
  • Contact intake with appropriate information on discharged patients in order to obtain MD order.
  • Follow-up with facilities, intake and /or liaisons for patients who are in the hospital or rehab. ยง Mail out All Care Keeping Connected information and any add'l information needed for discharged patients.
  • Secure 10+ Medicare admits per month
  • Keep updated lists, follow-up information and notes on all calls made.
  • Any other responsibilities assigned by supervisor.

QUALIFICATIONS Experience, Education and Licensure High School diploma or GED. Completion of course in medical records terminology preferred. Must be familiar with MS Word, Outlook and Excel. Unless otherwise agreed upon by employer, employee must have a valid drivers license. Furthermore, employee shall have use of a vehicle during working times and insurance as a driver of that automobile.

Language Skills: Ability to read and write basic sentences and to converse intelligently with staff.
