POSITION SUMMARY: Responsible for generating sales and referrals for All Care VNA.
Coordinate home care service plans for referred patients. Educate and act as a resource to physicians, discharge planners, case managers and clients. Act as a consultant to All Care staff regarding specific facility protocols and community services.

ESSENTIAL DUTIES AND RESPONSIBILITIES
· Responsible for increasing referrals to All Care VNA based on individual budgeted goals.
· Responsible for maintaining and growing present referral source relationships to meet budgeted goals.
· Responsible for establishing new referral sources that generate patients for All Care VNA to reach budgeted goals.
· Responsible for the marketing and promotion of all new services and programs developed for All Care VNA.
· Develops a database of qualified leads through referrals, telephone canvassing, face-to-face cold calling and networking opportunities.
· Any other responsibilities assigned by supervisor.
Collaborate with referral source to assist in the establishment of home care plans by providing one or more of the following services:
-Participation in-patient care rounds and/or case conferences as needed.
-Participation in family/facility meetings as needed.
-Evaluation of clients in Acute/Rehab settings to determine initial home care services and safety issues.
-Communicate to the medical profession, the community and All Care employees via
formal/informal presentations, written reports, etc. to facilitate timely quality care.

QUALIFICATIONS
Experience, Education and Licensure

Preferred 5 years combined sales experience preferable in acute and home care settings. Must have excellent presentation and communication skills, proven problem-solving skills and the ability to work independently. Must be familiar with MS Word, Outlook and Excel.
Unless otherwise agreed upon by employer, employee must have a valid drivers license. Furthermore, employee shall have use of a vehicle during working times and insurance as a driver of that automobile.

Language Skills
Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community.
