Shift:Friday, Saturday, and Sunday (8 hrs/day)

JOB PURPOSE: Community Health Nurse to provide quality nursing care to identified home health client’s in their home.  The scope of nursing includes, direct client care, advocacy, and communication with the primary nurse.  Client includes individual and family/significant other.  Weekend and holiday commitment as per agency policy.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Case Management
Continues the care plan designed by the primary nurse and documents specifically to provide for continuity of care

Communicates with primary nurse and/or coordinator(s) by providing copies of all visit documentation.  Urgent issues are addressed via telephone to coordinator on Monday at 8:00am

Ensures that new physician orders are accurately recorded, implemented and evaluated for client response.  Physician telephone orders are followed up by speedy memo to be left in Clinical Coordinators mailbox.


Demonstrates ability to organize and set priorities to accomplish work in a timely manner.

Attends team meetings when scheduled.


Patient Care
Implements nursing interventions to meet client needs and changed conditions

Participates in the planning, implementation and coordination of health teaching and counseling of client’s family or significant other.

Initiates appropriate referrals for additional services or community resources.         

Communicates clearly and accurately with other members of the health care team and with the client/family.

Professionalism
Complies with infection control and safety measures in accordance with the Agency’s policies and procedures. Initiates appropriate referrals for additional services or community resources.

Maintains high level of clinical expertise necessary to meet the needs of the client population and recognizes need for ongoing professional education and development within the Agency and in the community

Shares professional experience, skills and knowledge with other staff members.

Continually reassess client progress towards goals and expected outcomes, and documents this at every visit.  Collaborates with CHN to revise care plan to meet client’s needs and changing conditions.

Documentation
Completes and submits all assigned admission and revisit paperwork by 8:00 a.m. Monday.

Clearly documents nursing intervention and patient response to same on clinical record form.

Assesses client condition on every home visit.  Documents any change in client status from previous visit.

Documents goals and outcomes outlined on the plan of care established with the client, the family and the physician.       

Completes and submits documentation on a timely basis with clear, legible and concise information.

Completes documentation in accordance with Federal and State regulations and Agency’s policy.                          

Any other duties/responsibilities assigned by supervisor

QUALIFICATIONS
Experience, Education and Licensure
Graduate of a school of practical nursing accredited by the National League for Nursing or National Association for practical Nurse Education.  Current licensure in all states where client services are rendered.  Two years of medical-surgical nursing experience in an acute care setting or one year acute medical-surgical and one year community health experience.  Current Drivers license.

Language Skills
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
