The position will require this Manager to be in the North Andover office 2 days a week and the Lynn Office 2 days a week.  Manager will be required to rotate between the 2 offices.

POSITION SUMMARY: Responsible for directing, coordinating and evaluating the delivery of quality care in the home and community setting.

MANAGEMENT OF STAFF · Maintains expected productivity for each staff member · Maintains consistent caseloads for each staff member · Completes field supervisions timely · Documents counseling/disciplinary actions · Develops working relationship and positive attitude within the team · Monitor orientation process of new team member · Effectively communicates with staff in a positive manner · Maintains a schedule for meetings and conferences with staff. · Evaluates staff's skills and competencies and intervenes as appropriate · Monitors and assures timely computer transfers · Oversees the clinical schedule and holiday planning · Oversees the annual mandatory requirements · Possess leadership skills and supports agency's goals and values · Handles confrontative situations with courtesy and tact

MANAGEMENT OF PATIENT · Oversees Care Coordination weekly to assure appropriate case mix weight: o Case conferences a minimum of bi-weekly with each staff member o Assure appropriate visit volume o Assure appropriate HHA usage o Assure appropriate skill provided o Assure appropriate rehab visits are completed including reassessments o Assures appropriate recerts and discharges o Assures positive outcomes o Monitor and assure timely completion of incomplete documents o Customer service for patient/family problems and complaints

OPERATIONS · Interview and hire · Reports-Monitors reports and takes corrective action: o Payroll report-daily corrections and weekly signoff o Oasis Assessment Pending Report-weekly o LUPA/Therapy LUPA reports o Incomplete Document Report-weekly o Productivity Report · Complete evaluations timely · Attends Meetings o Weekly Manager's Meeting o Monthly Strategic o Monthly QI Meeting o Monthly Paraprofessional · On-Call as scheduled-Documents in on-call log all calls and responses. Patient related calls documented in an HMS note · Complete Non Medicare Reviews daily · Interdepartmental communication is prompt and professional · To assure staff completes all documentation per agency policy · Correcting documentation in IT system · Other duties and responsibilities assigned by supervisor

Delegates appropriately, support the organization goals and values · Enforces safety measures of employees and reports any incidents promptly · Any other task assigned by supervisor. · Check company email on normally scheduled days at a minimum of the beginning and ending of their normal scheduled day and responds ASAP but a maximum of within 24 hours · Provides effective oversight of patients on caseload with focus on meeting and/or exceeding agency quality outcomes, included but not limited to: o Case Mix Weight o Re-Certification rate o Target visits per episode o Acute Care Re-Hospitalization PATIENT CARE § Provides oversight in manner that assures above average patient satisfaction outcomes as measured by HHCHPS and in accordance with professional standards of practice. § Adheres to Agency established protocols for patient care (CHF protocol, Complex Wound Care, etc.)

QUALIFICATIONS Experience, Education and Licensure Graduate from an accredited NLN approved school of nursing, BSN preferred, or Graduate of an accredited school with major area of study in rehabilitative discipline, both with current registration in the State of Massachusetts. Previous nursing experience in home care with one or two years supervisory experience, or Three years of experience as a staff therapist with one year preferred in community health experience. Demonstrates effective written/verbal communication skills including excellent organization and leadership skill and skills in conflict resolution. Must be familiar with MS Word, Outlook and Excel.

Language Skills Ability to read, analyzes, and interprets general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
