JOB PURPOSE:
Responsible for directing, coordinating and evaluating the
delivery of quality care in the home and community setting.

ESSENTIAL DUTIES AND RESPONSIBILITIES
- Establishes department/individual priorities and
maintains management control of the daily operations to
ensure that efficiency, effectiveness and quality
standards are maintained.
- Ensures consistent and concise lines of authority and
responsibility, and cooperates and collaborates with
Agency staff members.
- Maintains a schedule for regular meeting and
conferences with staff.
- Communicates information regarding changes in
operations, policies and procedures to staff in a timely
manner.
- Implements and monitors staff compliance with all
Agency policies and procedures as they apply to his/her
department.
- Analyzes utilization of staff, adjusting caseloads as
necessary.
- Maintains current knowledge of care requirements for
all clients served by his/her team.
- Assesses client and family responses to care delivered
by his/her team.
- Works collaboratively within an interdisciplinary
model to ensure effective coordination of client care.
- Continually monitors the ability of staff to render
appropriate care in changing circumstances; takes
corrective action to address identified deficits.
- Evaluates the individual capabilities of each member
of his/her staff, and takes appropriate action if
discrepancy is recognized between the capability and
performance requirement.
- Identifies employee performance problems, provides the
appropriate counseling and instruction as needed. May
initiate disciplinary action with the approval of the
Vice President of Clinical Services/ Hospice
Administrator.
- Administers direct care to select clients as
needed to maintain own clinical skills and/or to assess
a particular client's requirements.
- Assists in identifying long range staffing needs of
branch office.
- Interviews, selects and hires personnel for his/her
team with the approval of the Vice President of Clinical
Services/ Hospice Administrator
- Participates in the development, implementation and
monitoring of a Quality Improvement Program which
includes but is not limited to: client and discipline
interview, record audit, standards of care, utilization
review, and performance evaluation.
- Participates in community groups, professional groups
and organizations as related to the provision of Hospice
Care.
- Cooperates with Federal, State and local agencies as
required and promotes and maintains an Agency
environment that is in compliance with Federal, State
and local regulatory agencies.
- Assists in the development of Agency policies and
procedures.
- Implements a system for regular monitoring of client
medical records in order to review appropriateness of
documentation in accordance with certification
requirements. (POC, HHA, Care Plans, etc.)
- Recommends and evaluates staff development activities
by identifying educational and training needs of his/her
staff.
- Participates in the ongoing educational program of the
Agency.
- Participates in the ongoing educational program of the
Agency.
- Participates in the education and orientation of
Hospice staff and volunteers.
- Demonstrates personal and professional growth and
development through participation in educational
programs and review of current literature regarding
health care and industry trends.
- Provides timely written performance evaluation for
his/her staff.
- Investigates and adjusts complaints from clients,
family, staff and other providers, reporting and/or
referring to, when necessary, the Vice President of
Clinical Services/ Hospice Administrator for
interventions.
- Represents Hospice, the Clinical Division and the
Agency by maintaining active involvement with other
community organizations and professional groups
- Any other duties/responsibilities
assigned by supervisor.

QUALIFICATIONS
Experience, Education and Licensure Graduate from
an accredited NLN approved school of nursing, BSN preferred,
or Graduate of an accredited school with major area of study
in rehabilitative discipline, both with current registration
in the State of Massachusetts. Previous nursing experience
in home care with one or two years supervisory experience,
or Three years of experience as a staff therapist with one
year preferred in community health experience. Demonstrates
effective written/verbal communication skills including
excellent organization and leadership skill and skills in
conflict resolution
Language Skills
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
