POSITION
SUMMARY: Evaluate and treat clients in the community setting
and responsible for
supervising PTA's and HHA's who are providing care under
his/her authority in accordance with
physician's orders to aid in the client's recovery and
rehabilitation. Per Diem Case manager is a responsible
member
of the team who works no less than 4 days/ week and carries
a caseload which is equal to a staff members.
Time
off requests are given to the manager for approval as per
agency policy. Weekend and holiday commitments are
consistent with staff clinicians. There is no daily
productivity requirements. Team meetings are required and
paid at the visit rate/ hour.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Case Management

Collect data and complete clinical assessment in a timely manner, formulate a plan of care using the identified client problem list, based on assessment.

Provide care in accordance with professional standards of practice guide for professional conduct, code of ethics.

Coordinate with and supervise HHA's as appropriate in accordance with Agency policy to assure the best patient outcome.

Maintain ongoing client's assessment and documentation in clinical progress notes following Agency policy.

Participate in case conferences with other disciplines regarding client care and outcomes.

Review and modify discharge plan in collaboration with client/family/significant other, other services involved and the client's physician.

Review with client's physician information regarding the client's status and discharge plan.

Attend team meetings.
Patient Care
-
Update and revise the client Plan of Care and expected outcomes to meet
changes in the client's status and the effectiveness of care being provided
by all disciplines.

Direct clinical interventions to meet the client's needs and changing condition.

Continually reassess the client's progress towards expected outcome and
document this at every visit.

Participate in the planning, implementation and coordination of health
teaching and counseling of client's family or significant other.

Initiate appropriate referrals for additional services or community resources.

Evaluate and revise as necessary teaching plans established to assist client
to reach established goals.

Communicate clearly and accurately with other members of the health care team
and with the client/family.

Comply with infection control and safety measures in accordance with the Agency's policies and procedures.

Maintain high level of clinical expertise necessary to meet the needs of the
client population and recognize need for ongoing professional education and
development within the Agency and in the community.

Instruct Home Health Aide and document teaching and outcomes on all cases
where Rehab professional and HHA work.
Documentation

Document client's level of understanding and response to teaching.

Document goals and outcomes outlined on the plan of care established with
the client, the family and the physician.

Complete and submit documentation on a timely basis with clear, legible and
concise information.

Complete documentation in accordance with Federal and State regulations and
Agency policy.

Any other duties/ responsibilities assigned by supervisor.
QUALIFICATIONS
Experience, Education and Licensure
Graduate of a Physical Therapy educational program
accredited by the Commission on Accreditation of Physical
Therapy Education (CAPTE). Licensed to practice in the state
where services are rendered. One year of experience as a
staff Physical Therapist under qualified supervision in a
hospital, clinic or certified home care agency. Current
driver's license.
Language Skills
Ability to read and interpret documents such as safety
rules, operating and maintenance instructions, and procedure
manuals. Ability to write routine reports and
correspondence. Ability to speak effectively before groups
of customers or employees of organization.
