JOB PURPOSE: To responsibly accept, screen and coordinate referrals to all services
within the Agency.

ESSENTIAL DUTIES AND RESPONSIBILITIES
- Accepts calls from all referral sources identifying
service needs.
- Assists referral sources in evaluating service needs
of client/family.
- Processes detailed information from referral sources
/client/family/referral source prior to initial home
assessment to enhance intervention.
- Evaluates crisis calls for immediate intervention as
needed.
- Interprets Agency policy and capacity to meet requests
for services.
- Explains intake and assessment process to
client/family/referral source interpreting scope of
services available.
- Seeks and maintains current operating knowledge of
other community resources making referrals as indicated
to ensure comprehensive home care. ยง Assures
communication and flow of information between Admissions
office and all clinical areas.
- Keeps Director informed of volume and flow of
referrals to ensure Agency's ability to respond
responsibly to service requests.
- Participates in the orientation program for new staff.
- Answers all calls from answering service.
- Takes referrals and MD orders/lab results during
non-business hours and relay information to appropriate
dept. in the a.m.
- Triage patient calls to visiting on call nurse,
hospital or notifies day supervisor of problems and need
for the next day visit.
- Documents all calls on log and the status of the call.
- Documents on clinical narrative any nursing
information or teaching done to patients.
- Documents any patient and or family concerns or
complaints on incident reports.
- Any other duties/responsibilities assigned by
supervisor.
QUALIFICATIONS
Experience, Education and Licensure
Graduate of an NLN accredited school of nursing.
Current Driver's License. Current RN license in the
state where client services are being rendered.
Baccalaureate degree in nursing preferred. Two years
of progressive experience within the field of community
health
nursing.
Language Skills
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
