Position Summary:Coordinate hospice care service plans for referred patients. Educate and act as a resource to physicians, discharge planners, case managers and clients. Act as a consultant to All Care staff regarding specific facility protocols and community services.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Collaborate with referral source to assist in the establishment of hospice care plans by providing one or more of the following services: -Participation in patient care rounds and/or case conferences as needed. -Participation in family/facility meetings as needed. -Evaluation of clients in all settings to determine initial hospice care services and safety issues. · Responsible to provide informational sessions to: patients, families and physicians and accept a patient on service when appropriate. · Responsible to keep current with criteria appropriate for hospice admissions.
  • Responsible for maintaining present referral source relationships and the establishing of additional referral sources.
  • Responsible for the marketing of all new services and programs developed for All Care Hopice
  • Responsible for the marketing and promotion of all services available through All Care Hospice
  • Communicate to the medical profession, the community and All Care employees via formal/informal presentations, written reports, etc. to facilitate timely quality care.
  • Maintain all necessary records and/or files including telephone logs.
  • Participates in community/industrial health promotion programs.

QUALIFICATIONS
Experience, Education and Licensure
Preferred 5 years combined experience in acute and home care settings. Must have excellent presentation and communication skills, proven problem-solving skills and the ability to work independently. Current driver's license.

Language Skills
Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community.
