POSITION SUMMARY: Implement the nursing care plan developed by or in collaboration with the community health nurse. The LPN evaluates and documents the client's response to nursing care.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Case Management Participate in case conferences with other disciplines regarding client care and outcomes.
Implement nursing interventions in a safe and therapeutic manner with sound knowledge of underlying behavioral and physical science concepts. Assist in the development of a discharge plan for each client in collaboration with community health nurse, client, family or significant other and other disciplines involved and the client's physician.
Inform physician of pending discharge plans and client status on discharge.
Patient Care Teach and counsel clients and family members on specific care needs between visits on promotion or maintenance of health.
Communicates clearly and accurately with other members of the health care team and with clients and their families.
Professionalism Complies with infection control and safety measures in accordance with the Agency's policy and procedures.
Continually reassesses client progress towards goals and expected outcomes and documents this at every visit; collaborates with community health nurse to revise care plan to meet client's needs and changing conditions.
Recognizes clinical strengths and weaknesses and seeks
out appropriate consultation.
Participates on Agency committees and agency's Quality Improvement activities.
Maintains high level of clinical expertise necessary to meet the needs of the client population and recognizes need for ongoing professional education and development within the agency and in the community.

Documentation Maintain appropriate documentation of nursing interventions and client response. Completes documentation in a timely manner and in compliance with Agency policy and procedures.
Submits accurate documentation on a timely basis.
Any other duties/responsibilities assigned by supervisor.
QUALIFICATIONS Experience, Education and Licensure Graduate of a school of practical nursing accredited by the National League for Nursing or National Association for practical Nurse Education. Current licensure in all states where client services are rendered. Two years of medical-surgical nursing experience in an acute care setting or one year acute medical-surgical and one year community health experience. Current Drivers license.
Language Skills Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
