Hospice Home Health Aide Paraprofessional Lynn - Job Code # PL3 Location: Lynn, MA HOURS/SCHEDULE:
20 Hours/week Position Summary: Assists clients with personal care and home management as instructed by the professional and documented on the written plan of care. AGENCY EXPECTATION OF EMPLOYEE ESSENTIAL DUTIES AND RESPONSIBILITIES Supervises and assists clients with Activities of Daily Living with demonstrated skill: - Bath (Including Bed Bath, Sponge Bath, Tub Bath, Shower, and Skin care) - Bed making (Both occupied and unoccupied beds) - Elimination (bedpan, urinal; as applicable) - Personal Hygiene (Including Nail Care, Mouth Care, Dentures Care, Shampoo in Sink, Shampoo in Tub, and Shampoo in Bed, Hair Care, and Shaving; as applicable) - Vital Signs (Including Pulse, Respiration, and Temperature) - Transfers (Including Bed to Chair/Wheelchair/Commode and with use of Hoyer Lift) - Ambulation (Including Walking Assisted with Cane/Walker and Unassisted) - Range of Motion (Both Active and Passive) - Positioning - Bag Technique - Other Areas as Designated by Agency Maintains a clean, neat, and safe environment for the client. Observes and reports any changes in the client's condition and environmental situation. Professionalism Attends inservices regularly. Adhere to Standard Precautions and infection control measures. Maintains equipment and supplies as appropriate. Provide care and comfort for the client. Allow client to make decisions and choices. Allow the client to be as independent as possible while maintaining safety Allow the family to interact with the client while providing care Be attentive to the clients needs Provide emotional support by being present and available for the client Maintain client safety Communication Communicates with scheduling staff, supervisors and clients, especially any changes or variation in schedule Document home health aide services provided and submit paperwork in a timely manner Maintain confidentiality Do not discuss personnel experiences with the client Identify the clients non-verbal communication in relation to pain and report to the supervisor Maintain a good understanding about the grieving process Accept clients wishes Any other responsibilities assigned by supervisor QUALIFICATIONS Certificates, Licenses, Registrations AMERICANS WITH DISABILITY SPECIFICATIONS While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee must regularly lift and/or move 50 pounds and frequently lift and/or move up to 100 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
