Shift: Weekends (Sat/Sun 8am-8pm)
Position Summary: Assists clients with personal care and home management as instructed by the professional and documented on the written plan of care.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Accept client cases, where ever needed, to meet client and Agency needs.
Understand and follow client Care Plan as directed by primary clinician.
Supervise and assist clients with Activities of Daily Living with demonstrated skill:
- Bath (Including Bed Bath, Sponge Bath, Tub Bath, Shower, and Skin care)
- Bed making (Both occupied and unoccupied beds)
- Elimination (bedpan, urinal; as applicable)
- Personal Hygiene (including Nail Care, Mouth Care, Dentures Care, Shampoo in Sink,
Shampoo in Tub, and Shampoo in Bed, Hair Care, and Shaving; as applicable)
- Vital Signs (Including Pulse, Respiration, and Temperature)
- Transfers (Including Bed to Chair/Wheelchair/Commode and with use of Hoyer Lift)
- Ambulation (Including Walking Assisted with Cane/Walker and Unassisted)
- Range of Motion (Both Active and Passive)
- Bag Technique
- Other Areas as Designated by Agency
Observe and report any changes in the client’s condition and environmental situation.
Encourage clients in self-help to increase their level of independence.
Maintain a clean, neat, and safe environment for the client.
Arrive on time, prepared to perform duties.
Attend inservices regularly.
Adhere to universal precautions and infection control measures
Maintain equipment and supplies as appropriate.
Communicate with scheduling staff, supervisors and clients.
Document home health aide services provided and submits paperwork in a timely manner
Participates on committees and projects.
Any other responsibilities assigned by supervisor
Experience, Education and Licensure
Minimum of eighth grade education; high school diploma or GED preferred. Must be a mature person of at least 18 years of age who has successfully completed a Certified Home Health Aide Training Program (minimum of 75 hours), and has demonstrated competency.
Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.
Certificates, Licenses, Registrations
Must be Certified as a Home Health Aide in Massachusetts. Must have a valid driver’s license and valid car registration. Must attend in-service education, a minimum of 12 hours per Calendar year, which must include those designated as mandatory.
